What to send
- Patient face sheet — demographics and insurance
- History and physical, or the hospital discharge summary
- Signed physician orders, with the disciplines requested
- Current medication list
- Any relevant wound, catheter, or ostomy documentation
Disciplines we staff
Skilled nursing, physical therapy, occupational therapy, speech therapy, social services, and certified home health aide.
If a patient needs a discipline we cannot staff for their location, we will tell you when you call rather than after we have accepted the referral.
What happens after you send it
We verify coverage
We confirm the patient’s insurance or Medicare eligibility and confirm the address falls within our service area.
We contact the patient
We call to introduce ourselves and schedule the start-of-care visit at a time that works for the patient and family.
We report back to you
The plan of care goes to the ordering physician, and we keep them updated on the patient’s progress.
