Home health is short-term, skilled care after a hospital stay, a surgery or a change in health. It is provided in your own home, on a schedule set by your physician.

• Skilled Nursing

  A registered nurse provides skilled care and instruction to patients and families, so that both can function independently at home.

• Physical Therapy

  Helps restore strength, balance and mobility after surgery, injury or illness, and works on reducing fall risk at home.

• Occupational Therapy

  Rebuilds the ability to manage everyday activities — dressing, bathing, cooking — and adapts the home to make them safer.

• Speech Therapy

  Addresses difficulty with speech, language, or swallowing following a stroke, surgery or neurological condition.

Paying for care

Three ways care gets paid for.

• Insurance Coverage

  We will verify your coverage upon receipt of any assignment of benefits, and we will bill your insurance carrier directly.

• Medicare

  If you are eligible for home health coverage through Medicare, our office will bill Medicare directly.

• Direct Payment

  When billed directly to you, the Wholesome Home Health statement clearly identifies the visits and hours spent by our care professionals. You pay only the per-visit or per-hour rate — we cover all taxes and insurance for our employees.
