Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Nondiscrimination
Wholesome Home Health, Inc. complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), sex, age, or disability.
Wholesome Home Health provides, free of charge and in a timely manner:
- Reasonable modifications for people with disabilities, and appropriate auxiliary aids and services — including qualified sign language interpreters and written information in other formats — so that people with disabilities can communicate with us effectively.
- Language assistance for people whose primary language is not English, including qualified interpreters and information written in other languages.
To ask for any of these, call (747) 262-3980. There is never a charge, and you are never asked to bring your own interpreter.
Free language assistance
Our staff speaks English, Armenian, Russian, Spanish and Persian. If you need an interpreter for any other language, ask and we will arrange one at no cost to you.

How to complain
Both notices above are enforced by the same office, so one complaint route covers privacy and discrimination alike.
